Things you'll need to bring to your appointment:
- Insurance card and/or billing information
- Name of primary care physician
- Please bring all relevant information including laboratory results, x-rays and previous doctors' reports
- Names of all medicines you're currently taking
- Names of medications to which you are allergic
- Know what medications you must avoid if allergy skin testing is to be performed
- Please fill out the new patient forms in advance of your visit (patient registration form, allergy history, systems review and HIPAA form, etc.). This will ensure that all information the doctor needs will be present at the beginning of your office visit.
